We are recruiting 2 Post Diagnostic Dementia Link Workers to be based in Crown House, Greenock, working as part of the multi-disciplinary Older Peoples Mental Health Team in Inverclyde. These are fixed term posts until 31 March 2022.
The Dementia Link Worker provides Post Diagnostic Support for people who have received a diagnosis of Dementia. You will work with individuals and their families for a minimum of a year, providing flexible and proactive advice, information and support around aspects which will enable them to live well with Dementia for as long as possible.
Inverclyde was chosen as the implementation site for the Dementia Care Coordination project. The Post Diagnostic Support Link Workers will support developments and improvements in local service provision.
The successful applicant will have a recognised professional qualification in social care, social work, nursing, occupational therapy, community work, counselling or other related occupations to a minimum of SVQ Level 3 or Nursing grade 5.
