Alzheimer Scotland is proud that Scotland is the first any only country in the world to offer a guarantee of a minimum of one year’s post diagnostic support (PDS) to everyone who is diagnosed with dementia. Our Link Workers are key to delivering this guarantee.
Our 5-pillar model of post diagnostic support is flexible and person-centred, putting people living with dementia and their families at its heart.
The role
As Post Diagnostic Support Lead, you will provide leadership, caseload management and Support and Reflection to a cohort of Link Workers based throughout the East of Scotland.
Drawing upon the national PDS improvement framework, you will work collaboratively with colleagues in monitoring and reporting on the quality and performance of the delivery of high-quality post diagnostic support in our Localities.
You will work in close collaboration with peers and colleagues across all parts of Alzheimer Scotland, leading by example and exhibiting behaviours in a leadership style that places engagement at the heart of delivering the best outcomes for people with dementia and their carers. In particular, you will have a responsibility to nurture creativity and ensure Link Workers continue to develop and have opportunities to learn in an environment that is safe, supportive and collaborative.
The person
You’ll have experience of working with and providing services and support to people with dementia and their carers and a firm understanding of a person-centred approach to planning and assessment. Highly organised, with drive and enthusiasm, you will have the ability to produce clear and high-quality written reports and have experience of management and leadership in a social care, community care, social work or health setting. A recognised professional qualification in social care, nursing, social work or other related occupation as well as possession of a driving license are essential to this post.
