We need an enthusiastic, skilled and knowledgeable practitioner to join our team.
The Post Diagnostic Dementia Link Worker will be based within the Community Older People's Mental Health Team at Woodlands Resource Centre in Kirkintilloch and will support people with dementia across East Dunbartonshire.
The Post Diagnostic Dementia Link Worker, in partnership with members of a multi-disciplinary team, will support individuals with dementia, their families and friends from diagnosis onwards. Implementing Alzheimer Scotland's 5 Pillars Model of post diagnostic support, you will provide person-centred support and information to people with dementia and their families. The work you do will enable people with dementia and their families to build resilience, plan and prepare for the future, develop coping strategies and subsequently enable them to live well with their diagnosis. The successful applicant will have the ability to work in a fast-paced and demanding role whilst providing high quality support.
