Job description
Dementia Link Worker - East Lothian
Contract: 1 post, 35 hours, temporary for 12 months
We are looking for a unique person who can support people with dementia and make sure that no one goes through dementia alone.
Dementia Link Workers are some of our most important people as they provide post diagnostic support to people who have received a new diagnosis of dementia
as well as their partners and families. You will provide information and support to people as well as signposting and connecting people to peer support or appropriate services for a minimum of one year. This is consistent with Alzheimer Scotland's 5 Pillars Model of Post Diagnostic Support enabling people to live well with dementia for as long as possible.
Dementia Link Workers enable people with dementia and their families and carers to be fully informed and plan their own pathway through dementia.
This Dementia Link Worker post is based in East Lothian and will cover all of East Lothian. You will be based in the Community Mental Health Team in Haddington.
Successful applicants will have a positive approach to dementia. You will also have good communication skills and a caring attitude. A willingness to learn and participate in training towards Enhanced Level of the Promoting Excellence Framework is essential.
This post will be subject to a criminal check through Disclosure Scotland.
